Advance Community Healthcare, a FQHC, we are recruiting for a full-time Psychiatric Nurse Practitioner who will provide services to our Southeast Raleigh, NC location.
The primary purpose of the Psychiatric Nurse Practitioner is developing and maintaining an outpatient behavioral health integrated practice, providing brief psychotherapy, treatment planning, behavioral evaluation, medication evaluation and management to a range of patients having mental illness, behavioral health concerns and substance abuse disorders under the supervision of a consulting psychiatrist. The Psychiatric Mental Health Nurse Practitioner reports directly to the Lead Medical Provider but is also expected to interact regularly with the behavioral health team and director.

Job Responsibilities
• Demonstrates competency with and provides individual and group therapy, medication assisted treatment (MAT). Monitors patients’ progress related to treatment and adjusts treatment accordingly.
• Conducts medication assessments, therapy and crisis response to new and existing patients. Develops individual service plans based upon assessment data and provided therapeutic interventions as necessary including the provision of on-going medication management services.
• Collaborate with behavioral health team and ancillary staff appropriately.
• Collaborates with our in-house team of behavioral health via internal referral and actively participates in regular clinical, provider and behavioral health team meetings.
• Participate in quality improvement activities, collaborate regularly with our Director of Behavioral Health, Program Managers, Lead Medical Provider.
Qualifications
• Experience providing Medically Assisted Treatment preferred.
• Two to three years practicing as a nurse practitioner preferably in a community health environment.
• One year experience providing psychiatric care in medical setting preferred.
• FNP in the state of NC; active and unrestricted DEA.
• Experience working with a Medicaid population is preferred.
• Experience working with a population that seeks challenges with social determinants of health, including mental health and substance abuse.
Working Hours
• Schedule: Monday – Friday, 8:00an – 5:00pm
