Our private practice, Bloomington Pediatrics is seeking a dedicated Pediatric Nurse Practitioner to join our healthcare team of 5 physicians and FNP in Bloomington, Illinois. Job Responsibilities

• Demonstrates excellent communication and coordination with the Care Team, families and other agencies regarding health status and needs of each patient.
• Provides health assessment for patients directed toward continuing a state of wellness, establishing the need for intervention, evaluating responses to and outcomes of therapy, and following a mutually agreed upon treatment plan.
• Utilizes standing orders established by the practice and Evidence Based Guidelines for effective and efficient patient care.
• Utilizes motivational interviewing techniques and patient coaching for patient self-management support.
• Proactively assists the Care Team in addressing the needs of the patient population.
• Assists in analyzing performance measurements to identify areas of improvement and works with the Care Team to establish and test methods for quality improvement.
• Performs patient education activities and distributes appropriate materials or resources.
• Attends and actively participates in Quality Improvement meetings.
• Performs various tests/procedures and medication administration independently in accordance with the licensed scope of care
• Interprets and evaluates findings of studies and tests
• Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow up.
• Communicates with physicians or other staff to ensure continuity of care.
• Writes prescriptions for medication and communicates with pharmacies in accordance with practice protocol.
• Knows where emergency equipment is located and makes it available to the provider.
• Accurately records the following during a visit: blood pressure, pulse, temperature, height, weight, records reason for visit as indicated, documents administration of any medications or immunizations and all procedures and obtains consent if applicable.
• Documents all patient communications in medical record according to policies and procedures.
• Takes an active role in achieving clinical integration goals by medical record preparation.
• Contributes to team effort by accomplishing related tasks as needed.

Qualifications

• Master’s Degree for Nurse Practitioner, specialization in Pediatrics preferred
• Current Illinois Registered Nurse (RN) license
• Current APN Nurse Practitioner License
• National APN Board Certification
• Current DEA and Controlled Substance license in State of Illinois
• Current CPR certification.
• Must be eligible as a provider for Medicaid.
• Clean malpractice history.

Working Hours

Office Hours are Monday through Friday from 8:00am to 5:00pm and rotating Saturdays from 8:00am to 12:00pm.
