Requirements:

• Board Certified (NCCPA or AANP) – REQUIRED
• Active VA license – HIGHLY PREFERRED
• BLS Certified – REQUIRED
• PROVIDER MUST HAVE EXTENSIVE SPINAL & INPATIENT EXPERIENCE
• PROVIDER MUST BE AVAILABLE FULL-TIME
• DEA – Verbal Confirmation Required at Name Clear
