Nurse Practitioner – Neurology/Stroke

Location: Modesto, California

Full-Time Days

Position Summary:
Join a leading healthcare team dedicated to advanced stroke care. Reporting to the Stroke Program Coordinator, the Stroke Program Nurse Navigator oversees multidisciplinary care, ensures compliance with certification requirements, provides patient education, and supports post-discharge follow-up. Collaborate with medical staff and stakeholders to drive performance improvement and deliver evidence-based care.

Key Responsibilities:

  • Liaise between medical staff, nursing teams, and families.
  • Coordinate care and oversee stroke program performance.
  • Conduct patient education and post-discharge follow-up.
  • Collaborate on program certification and improvement projects.

Qualifications:

  • Current CA RN and NP licensure with furnishing number.
  • DEA furnishing certificate.
  • BLS, ACLS, NIHSS (within 3 months of hire).
  • ACLS (within 3 months), ENLS (within 6 months).
  • CNRN or NVRN required; SCRN preferred.

Compensation and Benefits:
