We seek a Nurse Practitioner / Physician Assistant who has extensive experience working with patients who suffer from neurological conditions and those who present symptoms like seizures, headaches, paralysis, poor coordination, muscle weakness, attention issues and varying levels of consciousness. As part of an interdisciplinary care team, this position will provide telehealth and onsite visits to post acute facilities in the Dallas, Houston or San Antonio, TX area to help diagnose, treat, and manage patients suffering from brain, spinal and/or neurological trauma.
DUTIES AND RESPONSIBILITIES, may include but not be limited to
- Provide patient care to patients diagnosed with various neurological conditions, including but not limited to acute ischemic stroke, ICH, and Seizure disorders.
- Coordinate care plans, address patient needs, evaluate the effectiveness of care, and assess changes to the patient's medical condition to ensure healing outcome goals.
- Order diagnostic tests and studies and prescribe medications as needed.
- Patient-centered care and treatment management, bedside procedures, quality improvement activities, education, and teaching
ESSENTIAL ABILITIES
- Detailed knowledge and experience in neurological conditions and neurosurgical treatments;
- Ability to help diagnose, treat, and/or manage a wide range of conditions including stroke, intracranial hemorrhage, tumor, traumatic brain injury, spinal cord injury, cerebrovascular, neurodegenerative, neuromuscular and seizure disorders
- Help patients manage chronic conditions such as back pain or to manage post-surgical follow-up care.
- Ability to provide both telehealth and onsite visits to skilled nursing facilities near your location in the Dallas, Houston or San Antonio, TX area. (75% telehealth and 25% onsite visits)
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday, 2 or more hours time block each time
- Excellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.
- Comfortable working independently and in collaboration with other medical team members in utilizing approved protocols, procedures, and expert consultation for our adult neurocritical care patients.
QUALIFICATIONS
- Master's degree in Nursing Licensure, Certification, and Registration
- Current, valid Florida APRN license Certification through the ANCC, AANP (or equivalent certification body)
- Current BLS required Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)
- Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm Monday through Friday.(in 2 or more hours time block each time)
- No weekends or evenings at this time.
- Neurology-focused clinical experience (3+ years required)
- Geriatrics and telehealth experience are highly preferred.

Our clinician-focused technology and resources will help you:
- Conduct clinical assessments and evaluations.
- Formulate treatment plans as needed.
- Prescribe medications with proper dosage and administration instructions.
- Oversee telemedicine patient flow and daily functions of the telemedicine shift.
- Maintain accurate electronic medical records of patients' contact details, medical history, prescribed medications, allergies, diagnoses, and progress.
- Maintain integrity, availability, and confidentiality of all PHI
- Oversee the reporting of diagnostic testing results, follow-up treatment, referrals, and recovery.
- Participate in quality assurance and care improvement initiatives.
- Adhere to HIPAA standards for information security, privacy, and confidentiality.
