Full-time Nephrology Nurse Practitioner
Location: Milford, Derry, and Nashua, NH
Monday-Friday - No Call
Key Responsibilities:
Conduct patient visits in the office setting.
Provide comprehensive care to dialysis patients at our partnering dialysis centers in Milford, Derry, and Nashua, NH.
Manage and monitor patient conditions, adjust treatment plans as necessary, and coordinate with the nephrology team for patient care.
Perform routine nephrology assessments, including physical exams, lab reviews, and medication management.
Maintain accurate patient records and documentation, including progress notes and treatment plans.
Collaborate with physicians and healthcare providers to ensure optimal patient outcomes.
Participate in patient education to support adherence to prescribed treatment and overall health management.

Qualifications
Nephrology and inpatient experience preferred. New graduates are encouraged to apply.
Excellent clinical judgment, communication, and interpersonal skills.
Ability to work independently while being part of a collaborative team.
Flexibility and adaptability to the dynamic needs of patients and healthcare facilities.
Working Hours
36 patient contact hours per week
4 hours of administrative time per week
No on-call requirements
