• Schedule: M-F eight hours daily.
• Evaluation of patients including but not limited to performing histories and physical examinations
• Develop and implement clinical care plans for each patient based on their specific needs and the entering of orders needed to implement the care plans
• Education of patients and their families as needed
• Timely maintenance of accurate and complete documentation within the electronic health record, currently EPIC. Documentation will include all information needed for appropriate billing of medical services rendered
• The follow-up of all laboratory and other patient related results
• The response to and management of patient related phone calls and other forms of communication
• Medication management including but not limited to new therapy, renewals and prior authorizations

• Current NYS Registered Nurse License required
• Current Nurse Practitioner Certification required
