A health services network in New York City is currently seeking an experienced, licensed Nurse Practitioner (NP) to join their staff in Manhattan as a Nurse Practitioner for their Mobile Program. In this role, the Nurse Practitioner (NP) will be responsible for working with the Physicians and community team to coordinate and support the participant's functional, clinical, and psychosocial needs, with an emphasis on independence and wellness.

About the Opportunity:

  • Schedule: Monday to Friday
  • Hours: 8am to 5pm (40hrs per week)
  • Setting: Field

Responsibilities:

  • Supervise the Nurse Practitioners (NPs) and oversee clinical staff at assigned nursing homes and sites
  • Conduct annual wellness assessments and manage acute and chronic conditions for participants
  • Coordinate care following hospital discharges, ensuring timely medication reconciliation and care transitions
  • Review medical history and develop comprehensive diagnostic lists for participants
  • Ensure accurate clinical documentation and improve care management processes
  • Educate participants, families, and staff on disease management and promote self-care
  • Participate in Interdisciplinary Team (IDT) meetings and manage high-risk cases
  • Provide urgent care, home visits, and emergency coverage as needed
  • Perform other duties, as needed

Qualifications:

  • 2+ years of Clinical Nursing experience
  • Graduate of an accredited Nurse Practitioner (NP) program
  • NYS Nurse Practitioner (NP) license
  • DEA registration
  • AANP and/or ANCC Board Certification
  • Experience working with the Frail and/or Elderly population
  • Experience in a Clinical Nursing Practice, In-Home Care, Care Management, and/or a related setting
  • Computer savvy
  • Solid assessment, clinical, and documentation skills
