Primary Care Nurse Practitioner

Location: Chesapeake, Virginia

Position Overview:

Chesapeake Internists Limited, a well-established internal medicine practice in Chesapeake, Virginia, is seeking a dedicated Primary Care Nurse Practitioner (FNP or AGNP) to join our team. This full-time position operates Monday through Friday, 8:00 AM to 5:00 PM, providing comprehensive primary care services to adult patients aged 18 and older.


Key Responsibilities:

  • Conduct comprehensive assessments and develop individualized care plans for adult patients.
  • Manage common acute and chronic conditions, including hypertension, diabetes, and respiratory illnesses.
  • Perform routine physical examinations and preventive health screenings.
  • Provide patient education on health maintenance, disease prevention, and lifestyle modifications.
  • Conduct women's health screenings, including Pap smears and pelvic exams.
  • Collaborate with physicians and other healthcare professionals to ensure coordinated care.
  • Utilize electronic health records (EHR) for accurate and timely documentation.

Qualifications:

  • Education: Board Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP)
  • Experience: Minimum of 2 years of post-graduate experience in a primary care setting.
  • Licensure: Active Virginia license or eligibility to obtain.
  • Skills: Strong clinical assessment, communication, and interpersonal skills.
