Field-Based Nurse Practitioner

Location: Philadelphia, PA

Job Summary:

We are seeking an Adult/Geriatric or Family Nurse Practitioner to join our home-based healthcare team in Philadelphia, PA. As a Field-Based NP, you will provide comprehensive in-home primary care to patients with chronic and complex illnesses, working closely with an interdisciplinary team to improve patient outcomes and reduce unnecessary healthcare costs. This role offers flexible scheduling with Monday-Friday hours and allows for a combination of remote work and in-home patient visits.

Key Responsibilities:

  • Conduct in-home visits for patients enrolled in the program, performing comprehensive evaluations and follow-up visits targeting chronic disease management and prevention.
  • Prescribe medications, order diagnostic tests, and coordinate care with the patient’s PCP and other specialists.
  • Educate patients and their families on chronic conditions, preventative care, and medication adherence.
  • Collaborate with an interdisciplinary team, including social work, nursing, pharmacy, palliative care, and behavioral health professionals.
  • Document patient encounters accurately in the EMR system and participate in virtual clinical meetings and training sessions.
  • Share on-call coverage with other team members, including limited weekend responsibilities.

Qualifications:

  • Graduate of an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Medicine.
  • Active, unrestricted NP license in Pennsylvania.
  • Minimum 1 year of NP experience in primary care, geriatrics, adult medicine, or home care
  • Current BLS certification required; ACLS preferred.
  • DEA license and State Controlled Substance License upon hire or within 3 months.
  • Valid state driver’s license and proof of yearly influenza, Hepatitis B vaccinations, and TB testing.
