We are seeking an experienced Nurse Practitioner or Physician Assistant with a strong clinical Infectious Disease background to join our team in Texas. You will help provide services to our post-acute patients and provide follow-up telehealth visits, as needed. Under the guidance of a supervising Infectious Disease Physician, and as part of an interdisciplinary care team, this position will provide telehealth visits as part of ongoing medical management and acute interventions for patients in the post-acute care setting. You will play an essential role in enhancing the well-being and assisting each patient return to their highest level of independence. DUTIES AND RESPONSIBILITIES, may include but not be limited to

  • Management and treatment of patients with infectious diseases
  • Coordinate the patient plan of care with the physician and the facility's clinical disciplines to address patient needs, evaluate the effectiveness of care, and assess changes to the patient medical condition
  • Establish plan of care and work within protocols established by the supervising physician.
  • Order diagnostic tests and studies and prescribe medications as needed.

ESSENTIAL ABILITIES

  • Ability to help diagnose, treat, and/or manage a wide range of infectious diseases
  • Be knowledgeable of infectious diseases and their necessary treatments
  • Ability to provide telehealth visits with geriatric patients in skilled nursing facilities in Texas
  • Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm CST Monday through Friday, 2 or more hours' time block each time
  • Excellent communication skills and ability to effectively communicate with patients, family members, and other medical professionals.
  • Organized and professional manner.

QUALIFICATIONS

  • Master's degree in Nursing Licensure, Certification, and Registration
  • Current, valid Texas APRN license Certification through the ANCC, AANP (or equivalent certification body)
  • Current BLS required Medicare NPI #, 855I and 855r on file (Independent NP Medicare Provider Number/PTAN#)
  • Must be able to commit to work a minimum of 10 hours per week, between the hours of 9am-5pm CST Monday through Friday.(in 2 or more hours time block each time)
  • No weekends or evenings at this time.
  • Infectious Disease-focused clinical experience (3+ years required)
  • Geriatrics and telehealth experience are highly preferred.
