Hospitalist Nurse Practitioner - Fully Remote Transition Care Management - PA

Guthrie is looking for an experienced NP to join its expanding team.

Highlights of the Opportunity

• Traditional Monday-Friday schedule with 8-hour days
• Remote position with occasional on-site coverage as needed
• Participate in the management of post-discharge care to patients treated by the Hospitalist team
• Collaborate with Hospitalists, nursing team, and central scheduling for continuity of patient care
• Opportunity for moonlighting if desired

Work-Life Balance

• Fully remote position
• Traditional Monday through Friday schedule
• No call, no weekends, no holidays

Candidate Requirements
