We are seeking a dedicated and compassionate Nurse Practitioner Hospitalist to join our healthcare team. As a Nurse Practitioner Hospitalist, you will play a vital role in diagnosing and treating patients, managing their overall health, and collaborating with other healthcare professionals to ensure comprehensive care.
Job Responsibilities
Diagnose and treat acute and chronic illnesses in various settings, including urgent care and memory care facilities.
Develop and implement individualized treatment plans based on patient needs.
Administer medications and monitor patient responses, ensuring adherence to best practices in medication administration.
Provide education to patients and families regarding health management, disease prevention, and treatment options.
Collaborate with interdisciplinary teams to optimize patient outcomes in areas such as geriatrics, hospice palliative medicine, and acute pain management.
Maintain accurate and up-to-date patient records in compliance with HIPAA regulations.
Utilize ICD-10 coding for accurate documentation of diagnoses and treatments.
Participate in quality improvement initiatives to enhance patient care services.
Qualifications
Proficiency in acute pain management techniques.
Experience in memory care settings is preferred.
Familiarity with psychopharmacology principles for effective medication management.
Solid understanding of geriatric care principles.
Ability to administer medications safely and effectively.
Excellent communication skills for effective interaction with patients, families, and healthcare teams.
