We are seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join an innovative Hospital at Home program, delivering high-quality inpatient-level care in the comfort of patients’ homes.
- Conduct comprehensive evaluations and assessments of acutely hospitalized patients.
- Collaborate with attending physicians and Command Center staff to develop and implement individualized inpatient care plans.
- Provide exceptional patient care in diverse settings, including hospitals, virtual platforms, and patients' homes.
- Full-time, day shift
Qualifications:
- Experience: Minimum of 2 years in inpatient advanced practice roles.
- Adaptability: Comfortable working in a dynamic and evolving environment.
- Clinical Skills: Strong clinical acumen with the ability to interpret and respond to complex cases effectively.
- Communication: Excellent verbal and written communication skills, conflict resolution abilities, and empathy to deliver compassionate, patient-centered care.
