We are excited to launch our Palliative Care Program and are seeking a Full time Nurse Practitioner to join our team in the Tampa area. The APRN will play a crucial role in providing comprehensive and specialized medical care of chronic patients. You will focus on preventing, diagnosing, treating, and coordinating care of patients with a wide range of illnesses and chronic conditions.

Job Responsibilities

1. Obtain health history and perform physical examinations
2. Diagnose and direct counseling for each patient on a plan for treatment
3. Prescribe medication in accordance with state statue and professional practice guidelines
4. Maintain a problem-oriented health record for each patient
5. Assure continuity of care by providing comprehensive medical services on a timely basis at patients home based on scheduled visits
6. Provide excellent internal/external customer service via telephone, videoconference, or chat

Qualifications

* Valid State Nurse Practitioner License
* 2 years experience in a palliative care as well as working with a multi cultural patient population
* Current BLS and ACLS certification
* Current State DEA Certificate is a must
* Bilingual (English/Spanish) preferred
* Certification by one of the Nurse Practitioner certifying bodies (ANCC or AANP) or Specialty certification in Palliative care such as CHPN/ACHPN is a plus
