- 2 years of recent experience in Palliative Care/Hospice required
- Schedule: Mon-Fri 8AM - 5PM
- Start date : ASAP, 6 month contract
- 10-15 patients per day (split virtual and in person)
- Onboarding will take 30 days to complete
- Support staff on site, full clinical team
