SSM Health Greater Fond du Lac Palliative Medicine team is searching for a full time Nurse Practitioner to join our team at SSM St. Agnes Hospital Home Care & Hospice. This provider will support our team by meeting with patients in the clinic settings and virtually. May also see patients in the hospital setting when cross coverage is needed. These visits will provide assessments and symptom management based on patient diagnosis and goals of care. Participation in weekly interdisciplinary team meetings and daily huddle will be part of this role. Ideal candidate has experience practicing in palliative care or hospice environment.

Minimum Qualifications

• Wisconsin APRN licensure as a nurse practitioner
• Active and unrestricted Wisconsin RN license
• Active and unrestricted Wisconsin DEA or the ability to obtain
• Experience in palliative or hospice care strongly preferred

• See patients in the Clinic setting and virtually. Provide hospital cross coverage when needed.
