Job Description
Become part of a community hospice organization as a Nurse Practitioner, collaborating closely with the Medical Director, Hospice Physicians, and Hospice and Palliative Care Teams to deliver compassionate care to patients and families across diverse settings.
The Position:
- 75% Outpatient Palliative Care. 25% Inpatient Hospice
- Preform clinical care which includes bedside evaluations; physical exams; radiological and laboratory interpretations; follow- up inpatient care; discharge orders, patient instructions and education; and participate in case management
- Provide services related to hospice and palliative care, assess patient needs and anticipate service requirements.
- Coordinates with social workers, case managers and applicable ancillary staff regarding hospice and palliative care services.
- Schedule is Monday – Friday 8:00 am to 4:30 pm
- Call (mostly phone): 1 day/week 4:30 – 7 am and one weekend a month from 4:30 pm on Friday to 7 am on Monday

Apply if you Have…
- An Active and Unencumbered Nurse Practitioner License in the state of Wisconsin
- A Master’s Degree in appropriate field of Nursing
- In field Nurse Practitioner experience
- Hospice and Palliative experience
- The ability to work independently, make accurate, and at times, quick judgements
- The ability to respond appropriately to crisis outside of a hospital setting
- Acceptance of and adaptability to different social, racial, cultural, and religious modes
