LTC Physician Care, PC is a new and emerging medical group dedicated to serving patients by providing Primary Care Physician (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan. Our goal is to improve the overall quality of care, patient outcomes, quality of life, refine facility processes, and enhance patient relationships.
LTC is seeking dedicated - Palliative Care - Advanced Practice Providers to join our team!

Requirements and Qualifications

  • Experience in Palliative Care, Primary Care, or Geriatrics is strongly preferred.
  • Candidates must hold a valid and unrestricted APP medical license in Michigan.
  • Candidates must hold a valid DEA license in the state of Michigan.
  • Candidates must have completed a valid U.S. APP certification program
  • Candidates must have completed an approved APP course of study from an accredited institution
  • Candidates must be able to satisfactorily complete a pre-employment background screening process (Criminal History, Licensing, etc.)
  • Candidates must pass a pre-employment drug-screen

Job Structure

  • This is an in-person position
  • Providers will commute directly between home and their assigned facility
  • APP works in coordination with an LTC Collaborating Physician
  • APP is responsible for the day-to-day care of their patients within the facility
  • After-Hours on-call is shared among all LTC Providers (currently a 15 week rotation)
  • Standard Monday-Friday 8a-5p workday – Full Time

Compensation and Benefits

  • Potential to exceed 125K per year.
    • Productivity Based Compensation Model (No Cap)

    • Clinical Standards and Quality Measures must be met.
    • 15 PTO days annually
    • $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance
