A well-established healthcare organization is seeking a compassionate Field NP to join their home care team. Based in Queens, this role involves providing care to patients in their homes.
Responsibilities:
- Providing comprehensive assessment, including multifactorial complex physical examination and psychosocial needs, self - management education & tools to better manage patient's multiple conditions
- Ensuring development of care planning and access to preventive care services in accordance with member and plan health outcome goals and objectives
- Acting as a clinical resource to the member, family, and interdisciplinary team
Qualifications:
- Active ANP, FNP or GNP license in the state of New York
- Experience with Home Care and/or Geriatrics
Desired Skills:
- Managed Care experience
- Experience in an Acute, Subacute, and/or Long Term Care setting
$130,000-150,000 base salary + 15k sign on or student loan assistance + comprehensive benefits
