Details:

  • Setting: primary care clinic for all geriatric patients
    • provide full spectrum of health care services, from primary to acute to long-term care for frail elderly individuals who are certified to require nursing home care
    • Provide primary medical coverage in collaboration with physicians as needed. Perform initial, semi-annual and annual assessments, perform additional assessment of patients as needed and implement appropriate plan of care
    • Participate in discharge planning for acute and long-term placement
    • Evaluate and treat patients during acute illness
    • Make nursing home or home visits when needed
    • Participate in provider on-call rotation as required
  • Schedule: Monday through Friday
    • May have some night and weekend call
    • 6-8 patients per day
    • Primarily work out of the Health Center along with interdisciplinary team
      • There is full coordinatation of patient visits, transporation for patients, and continuity of care amongst other types of providers

Required:

  • Active Alabama NP license
  • Active and unrestricted DEA and controlled substance registration (CSR/CDS)
  • Active AANP/ANCC certification
  • Current BLS certification
  • Minimum 5 years of NP experience required (preferably in long-term care)
  • Certification as a Nurse Practitioner with prescriptive authority required

Compensation:

  • W2 contract - paid weekly - pay negotiable
