Job Type: Part-Time (Onsite)
Shift: 3 Shifts per week (5 hours per day)
Role Overview:
We are seeking a dedicated and skilled Nurse Practitioner (NP) to deliver comprehensive primary care services to residents in assisted living facilities. This role offers a unique opportunity to work closely with an interdisciplinary team to enhance the quality of life for older adults in long-term care settings.
Key Responsibilities:

Conduct thorough health assessments, including physical exams, medical history reviews, and medication reconciliation.

Develop and implement individualized care plans in collaboration with an interdisciplinary healthcare team, addressing each resident's specific needs.

Manage chronic conditions and acute illnesses, providing timely interventions and referrals as necessary.

Order and interpret diagnostic tests, such as lab work and imaging, to aid in accurate diagnosis and treatment planning.

Prescribe medications and treatments within the NP scope of practice, adhering to state regulations and facility policies.

Initiate and participate in chronic care management services to enhance patient health outcomes.

Provide health education and counseling to residents and their families on disease prevention, health maintenance, and self-care strategies.

Maintain accurate and up-to-date documentation of assessments, treatments, and communications in the eClinicalWorks (eCW) electronic health records system.

Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall standard of care.
Requirements:

Master's degree in nursing and successful completion of an accredited Nurse Practitioner (NP) program.

Unrestricted New York State Nurse Practitioner license in good standing.

Active DEA permit to prescribe medications.

Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.

Minimum of 3 years of clinical experience, preferably in a long-term care setting.

3600 hours of clinical experience is required.

Primary care experience is preferred.
Preferred Qualifications:
- Prior experience working with geriatric populations in assisted living or long-term care facilities.
- Excellent communication and collaboration skills within an interdisciplinary team.
- Familiarity with eClinicalWorks (eCW) electronic health record system.
- Strong clinical decision-making and patient-centered care approach.
