Nurse Practitioner (Adult/Geriatric) - Amsterdam, NY

Job Type: Part-Time (Onsite)

Shift: 3 Shifts per week (5 hours per day)

Role Overview:

We are seeking a dedicated and skilled Nurse Practitioner (NP) to deliver comprehensive primary care services to residents in assisted living facilities. This role offers a unique opportunity to work closely with an interdisciplinary team to enhance the quality of life for older adults in long-term care settings.

Key Responsibilities:

  • Conduct thorough health assessments, including physical exams, medical history reviews, and medication reconciliation.

  • Develop and implement individualized care plans in collaboration with an interdisciplinary healthcare team, addressing each resident's specific needs.

  • Manage chronic conditions and acute illnesses, providing timely interventions and referrals as necessary.

  • Order and interpret diagnostic tests, such as lab work and imaging, to aid in accurate diagnosis and treatment planning.

  • Prescribe medications and treatments within the NP scope of practice, adhering to state regulations and facility policies.

  • Initiate and participate in chronic care management services to enhance patient health outcomes.

  • Provide health education and counseling to residents and their families on disease prevention, health maintenance, and self-care strategies.

  • Maintain accurate and up-to-date documentation of assessments, treatments, and communications in the eClinicalWorks (eCW) electronic health records system.

  • Participate in quality improvement initiatives and interdisciplinary meetings to enhance the overall standard of care.


Requirements:

  • Master's degree in nursing and successful completion of an accredited Nurse Practitioner (NP) program.

  • Unrestricted New York State Nurse Practitioner license in good standing.

  • Active DEA permit to prescribe medications.

  • Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.

  • Minimum of 3 years of clinical experience, preferably in a long-term care setting.

  • 3600 hours of clinical experience is required.

  • Primary care experience is preferred.


Preferred Qualifications:

  • Prior experience working with geriatric populations in assisted living or long-term care facilities.
  • Excellent communication and collaboration skills within an interdisciplinary team.
  • Familiarity with eClinicalWorks (eCW) electronic health record system.
  • Strong clinical decision-making and patient-centered care approach.
