Locum - NP - Geriatric Medicine in St. Joseph County, IN.

We are hiring a NP - Geriatric Medicine for a locum assignment in St. Joseph County, IN!

Job details:

• Start: ASAP
• Shifts: full time, 8a-4p M-F + rotating call one week on, one week off
• Duration: Six (6) months to start
• On Call coverage – one week on, and one week off.
• For on-call during the week it is from approximately 4:30 PM to 8:00 AM the next day. On Saturday and Sunday, it is the entire day. There is no requirement to go anywhere when Nurse Practitioners take call. They only need to be available by phone.
• Need Geriatric experience
• Provide primary medical management.
• Performs assessments in collaboration with the physician and as needed.
• Develops and implements appropriate plans of care to PO participants.
• Evaluates participant physical complaints and provides appropriate treatment.
• Provides participants and caregiver teaching and education.
• Functions as a member of the Interdisciplinary Team (IDT).
• Perform rounds in the POs contracted nursing homes.
• Ability to work with minimal supervision and exercise independent judgment.
• Must be comfortable functioning in a virtual, collaborative shared leadership environment.
• Able to communicate effectively with individuals and groups representing diverse perspectives.
• Ability to type with speed and accuracy. Ability to use other software as required to perform the essential functions of the job.
• Ability to perform multiple duties and functions related to daily operations and maintain excellent customer service skills.
• Must exhibit critical thinking skills and possess the ability to prioritize workload.

Job Requirements:

• Active IN License
• BLS, CDS, DEA
• Board Certified by ANCC or AANP
• A minimum of one (1) year working with the frail or elderly population strongly preferred
