  • As soon as credentialed - ongoing
  • 6 months with the possibility to extend

• Schedule

  • Monday to Friday, 20 hours per week, part-time hours

• Clinical Details

  • Provide primary medical coverage in collaboration with physicians as needed
  • Perform initial, semi-annual, and annual assessments of participants
  • Participate in discharge planning for acute and long-term placement
  • Evaluate and treat participants during acute illness
  • Prudently prescribe medications, therapies, and other treatments for participants
  • Make nursing home or home visits when needed
  • Following consultation with the Primary Care Physician refers participants to medical specialist as indicated
  • Work with Medical Director and Clinic Supervisor to formulate clinical policies, procedures, and standards of care
  • Advise the Medical Director and/or Primary Care Provider in ways and means to establish better accountability of the services to participants and referral sources

• Requirements

  • Master's degree in Nursing
  • Minimum five (5) years of experience required, preferably in long-term care
  • Active PA License
  • BLS, CDS, DEA
  • Board Certified by ANCC or AANP
  • Certification as a Nurse Practitioner with prescriptive authority
  • A minimum of one (1) year working with the frail or elderly population - Strongly Preferred
  • Must have a valid driver’s license, proof of insurance and have means of transportation
