• Schedule

• Full time: Monday - Friday, 8am-4pm
  • will also consider part time
• Rotating 24hr call: one week on, one week off
  • For on-call during the week: 4:30 PM to 8:00 AM the next day
  • On Saturday and Sunday, it is the entire day

• Clinical Details

• Coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community
• Patients in program: 55 years old or older
• Provide primary medical management in collaboration with the organization Physicians
• Perform assessments in collaboration with the physician and as needed
• Develop and implements appropriate plans of care to participants
• Evaluate participant physical complaints and provides appropriate treatment
• Provide participants and caregiver teaching and education
• Provide daily oversight of clinic
• Perform rounds in the contracted nursing homes
• Regularly informs the facility of the medical condition of each participant, remaining alert to pertinent input from other team members, participant’s caregivers, as well as documenting changes in a participant’s medical record consistent with documentation policies

• Requirements

• Active IN License
• BLS, CDS, DEA
• Board Certified by ANCC or AANP
• A minimum of 1 year working with the frail or elderly population - Strongly Preferred
• Geriatric experience required

• Call

• No requirement to go anywhere when Nurse Practitioners take call
• Only need to be available by phone
• May need to document what occurred from the call
  • Recommend having a pad & pen handy to make a note of what occurred if not in front of computer at the time

• Support Staff

• RN typically handles everything as they are also on call

• Currently TruChart but will be switching to Epic in March
