COMPENSATION:
- $125,000 to $155,000 Annual Income
- $105,000 to $125,000 Base Salary

LOCATION:
- Dayton, Ohio

This practice sees patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), and a very limited number of private homes. They are currently hiring in the Dayton area and will work with providers during the interview process to ensure a mutually-agreeable geographic territory in which all buildings will be located.


COMPANY PROFILE:
This organization is dedicated to delivering convenient, high-quality primary care to facility-based geriatric patients in the comfort of where they call home. They currently boast a team of more than 15 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.


POSITION DESCRIPTION:
The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients.

Nearly all patients are located in facilities (SNFs, ALFs, and ILFs), with a limited number in private homes (seeing patients in private homes is optional and NOT required).

In SNF settings (with no travel during the day), providers see an average of 15 to 16 patients per day.

When traveling between ALFs, ILFs, and/or homes, providers see an average of 9 to 12 patients per day (this is adjusted based on anticipated travel time).

Specific duties of the role include, but are not limited to:

- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and adjust treatment plans as necessary
- administer medications, treatments, and other therapeutic interventions as prescribed
- perform minor procedures and wound care as required (and as settings permit)
- monitor vital signs and patient responses to treatment
- educate patients and families on health management, disease prevention, and treatments
- provide guidance on lifestyle modifications and self-care practices
- work with physicians, nurses, and other professionals to coordinate comprehensive care
- ensure compliance with all relevant healthcare regulations and standards

The practice uses the eClinicalWorks EHR.

On-call is phone triage only with very low volume. Providers rotate the on-call responsibility, with a shift every 10 weeks or less.



REQUIREMENTS:
- Ohio NP license
- DEA
- advanced practice experience preferred, not required
- new grads with RN backgrounds in home health, trauma, or ICU are encouraged to apply
