Locum Nurse Practitioner – PACE Program (6-Month Assignment) – Mishawaka, IN

A leading healthcare organization in Mishawaka, IN is seeking an experienced Locum Nurse Practitioner for a 6-month assignment to provide care within a Program of All-Inclusive Care for the Elderly (PACE). This is an opportunity to work with a dedicated team providing comprehensive, coordinated care to the elderly, helping them maintain independence while receiving essential medical services.

Position Details:

• Title: Nurse Practitioner
• Duration: 6-month assignment to start (Full-time preferred; part-time considered)
• Schedule: Monday – Friday, with on-call coverage (see below)
• On-Call Rotation:
  • One week on, one week off
  • Weekdays: 4:30 PM – 8:00 AM (phone availability only)
  • Weekends: 24-hour phone availability (no in-person visits required)
  • RNs handle most calls; NPs may need to document as needed
• EMR System: Currently TruChart, transitioning to Epic in March

Job Responsibilities:

• Provide primary medical management in collaboration with PACE physicians
• Conduct assessments and develop treatment plans for participants
• Address participant physical complaints and provide appropriate treatment
• Educate participants and caregivers on health conditions and care plans
• Serve as a member of the Interdisciplinary Team (IDT)
• Provide daily oversight of the clinic and perform rounds in contracted nursing homes
• Regularly update the IDT on participants’ medical conditions
• Maintain detailed documentation in compliance with policies

Qualifications:

• Active Indiana NP License – Required
• Board Certification (ANCC or AANP) – Required
• BLS, CDS, DEA Certification – Required
• Minimum 1 year of experience working with the frail or elderly population – Preferred
• Strong communication, collaboration, and problem-solving skills
• Proficiency in Microsoft Office and EMR systems (TruChart, Epic preferred)
