We are seeking an experienced Geriatric Nurse Practitioner.  The facility offers continuous, quality care to the elderly, allowing them the independence of living at home.

• The primary purpose of your job position is to provide primary medical management in collaboration with the organization’s Physician(s).
• Performs assessments in collaboration with the physician and as needed.
• Shift(s) Description: Monday - Friday (NOT open to 4 days/week schedule) + On call
• Weekend/On-call Requirements: On Call: Required for after hours and weekend coverage
• Competitive salary range based on experience and training (to be discussed at interview). Provider may submit salary expectations with presentation.
• Must be ANCC/AANP Board-Certified
• Must have an active IN license (or willing to obtain)
• 5-7 years of NP experience, preferably in long-term care not including RN or training experience (note in "Experience" field)
