65 Prime Plus by Mercy is seeking Nurse Practitioners across Missouri with an interest in building relationships, caring for patients with complex needs, changing lives, and making a difference for seniors. This is an outpatient-only position, no call position with patients on Medicare and Medicare Advantage. Our 65 Prime Plus by Mercy team consists of 2 Physicians, 2 Advanced Practitioners, and an engaged support staff focused on quality patient care.

Job Responsibilities
• Value-based care – adequate time with patients, relationship building, quality incentives.
• 30 minute – 1 hour appointment slots for all patients – adequate time without rush
• Patient panel will offer intellectually stimulating and rewarding environment
• Dedicated panel nurse for telephone encounters, triage, and in basket support
• In-person, virtual, or telephonic encounters – annual comprehensive visits, transitional care, and same day visits
• Exceptional work/life balance
• Provide team-based care to manage a panel of 1,000 with collaborating physician allowing for longitudinal relationships with providers, patients and families
