We are seeking a Nurse Practitioner to join our Programs of All-Inclusive Care for the Elderly (PACE) team in Mishawaka, Indiana. This innovative program provides comprehensive care for individuals aged 55 and older who are certified to need nursing home care, enabling them to maintain independence in their community. This is a full-time position with the option for part-time, beginning with a six-month assignment. The NP will collaborate with physicians and interdisciplinary teams to deliver high-quality care to PACE participants both in clinical settings and remotely during on-call hours.

  • Conduct assessments in collaboration with PACE physicians and interdisciplinary teams.
  • Develop and implement care plans tailored to participant needs.
  • Evaluate participant complaints and provide timely, appropriate treatment.
  • Oversee daily operations at the PACE clinic.
  • Perform rounds at contracted nursing homes.
  • Educate participants and caregivers on health management and care plans.
  • Document participant health status and care activities in the EMR system (TruChart, transitioning to Epic in March).
  • Participate in on-call rotations (week on/week off), providing remote phone consultations as needed.


Job Responsibilities

Qualifications

Active Indiana License – Required.
Board Certification by ANCC or AANP – Required.
Certifications: BLS, CDS, DEA – Required.
Minimum 1 year of experience working with frail or elderly populations strongly preferred.
Experience with interdisciplinary team collaboration and geriatric care.
Proficiency in EMR systems; experience with Epic preferred.

Working Hours

Full-Time Schedule: Monday–Friday with on-call rotations (week on/week off).
On-Call Coverage:
Weekdays: 4:30 PM – 8:00 AM (phone availability only).
Weekends: 24-hour phone availability (no travel required).
Part-time arrangements may be considered.
