An innovative, growing, mission-based organization is seeking an experienced Nurse Practitioner to join our PACE (Programs for All-Inclusive Care for the Elderly) Clinical Team in South Boston, Massachusetts.


About the PACE Model of Care:

  • Our team collaborates to manage the care of frail elders, supporting both participants and each other in achieving care goals with small patient panels (65-75 patients per provider).
  • Comprehensive On-Site Care: Participants attend our center for Adult Day Health activities and receive on-site clinical care, including primary care, behavioral health, and rehabilitation services.
  • Continuity Across Care Settings: Nurse Practitioners follow participants across all care locations, including sub-acute, long-term care, and home settings. Regular home visits are encouraged, with dedicated time allotted for this effort.

Position Highlights:

  • Patient Panel: Manage 65-75 participants per provider within the PACE model.
  • Schedule: Monday through Friday, daytime hours.
  • 32 Hours of patient care and 8 hours of admin time each week
  • Option to work from home once a week for administrative tasks and/or note completion.
  • On-Call requirements include one weeknight every other week and one weekend every other month, plus one holiday per year.

Primary Care Duties:

  • Provide care at the PACE center, assisted living facilities, skilled nursing facilities, and participants' homes.
  • Conduct health assessments, evaluate patient needs, and develop comprehensive care plans.
  • Update Electronic Medical Records (EMR) with appropriate coding and documentation.
  • Precepting Opportunities: Wonderful opportunity to precept residents and students within the PACE program, if desired.

Compensation and Benefits:

  • Salary based on experience

Requirements:

  • Education: MSN in Geriatric Nursing or a related specialty field.
  • Licensure: Adult Gerontology or Family Medicine Nurse Practitioner certification.
  • Experience: 2+ years of experience working with frail or elderly populations preferred.
  • Mobility: Must be able to travel promptly to ESP sites, participants' homes, and affiliated care locations as needed.
