Our client in Chester county, PA is in need of a Geriatric Medicine Nurse Practitioner for a Locum assignment!

Job details:

  • Start - ASAP
  • Mon - Friday. 20 hours per week
  • Provide primary medical coverage in collaboration with PO physicians as needed. Performs initial, semi-annual and annual assessments, performs additional assessments of participants as needed and implements appropriate plans of care to PO participants.
  • Participates in discharge planning for acute and long-term placement.
  • Evaluate and treat participants during acute illness. Prudently prescribes medications, therapies and other treatments for participants.
  • Following consultation with the Primary Care Physician refers participants to medical specialist as indicated.
  • Make nursing home/or home visits when needed.
  • Work with Medical Director and Clinic Supervisor to formulate clinical policies, procedures and standards of care.
  • Assists with the development of standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
  • Advise the Medical Director and/or Primary Care Provider in ways and means to establish better accountability of the PO services to participants and referral sources.
  • Acts as resource during intake of new participants and in the day-to-day operation of the center programs.

Job Requirements:

  • Master's degree in Nursing - REQUIRED
  • Minimum Five (5) years of experience required preferably in long-term care.
  • Active PA License - REQUIRED.
  • BLS, CDS, DEA - REQUIRED
  • Board Certified by ANCC or AANP.- REQUIRED
  • Certification as a Nurse Practitioner with prescriptive authority required.
  • A minimum of one (1) year working with the frail or elderly population strongly preferred
  • Must have a valid driver’s license, proof of insurance and have means of transportation
