Job Description:

Seeking a Locum Nurse Practitioner (NP) with expertise in Geriatric Medicine for a 6-month assignment in Coatesville, PA. This is a part-time position (20 hours per week) with no weekend or on-call requirements. The role involves providing primary medical coverage for a geriatric population in collaboration with primary care physicians and medical staff.

Key Responsibilities:

  • Primary Medical Coverage: Collaborate with Primary Care Physicians (PCPs) to provide primary medical care to Program of All-Inclusive Care for the Elderly (PACE) participants.
  • Assessments: Conduct initial, semi-annual, and annual assessments for PACE participants. Perform additional assessments as needed.
  • Care Planning: Develop and implement appropriate plans of care for participants, adjusting care plans as necessary.
  • Acute Illness Care: Evaluate and treat participants during periods of acute illness. Prescribe medications, therapies, and treatments, consulting with PCPs as needed.
  • Specialist Referrals: Refer participants to medical specialists when necessary.
  • Nursing Home/Home Visits: Conduct visits to nursing homes or participants’ homes as needed.
  • Discharge Planning: Participate in discharge planning for both acute and long-term care placements.
  • Clinical Policies: Work with the Medical Director and Clinic Supervisor to develop and implement clinical policies, procedures, and standards of care. Monitor and evaluate patient care practices and service delivery.

Qualifications:

  • Education: Master’s degree in Nursing (Required).
  • Experience: Minimum of 5 years of experience, preferably in long-term care or geriatrics.
  • License: Active Pennsylvania Nurse Practitioner License (Required).
  • Certifications:
    • BLS (Basic Life Support) certification (Required).
    • CDS/DEA (Controlled Substance Registration/Drug Enforcement Administration) certification (Required).
    • Board Certified by AANP (American Association of Nurse Practitioners) or ANCC (American Nurses Credentialing Center) (Required).
    • Nurse Practitioner certification with prescriptive authority (Required).
  • Preferred Experience: Minimum of 1 year of experience working with frail or elderly populations.
