The Nurse Practitioner has the responsibility for providing direct and indirect care to the participants in the Health Clinic and Day Center. Works under the direct supervision of the Medical Director/PCP. Manages complex clinical situations for which he/she is responsible.
Job Responsibilities
• Initiates, evaluates, reviews and changes orders for selected medications and diagnostic studies/treatments within attending physician approved protocols.
• Assesses, initiates, and coordinates participant care on a daily basis under the supervision of the attending physician.
Makes referral to other physicians including specialists with staff PCP approval.
• Requests consultations from other services as needed in collaboration with attending physician and/or established protocols.
• Acts as liaison with physicians, nurses and other healthcare professionals providing education, communication and support for healthcare staff, participants, and families. Directs participant management in conjunction with physician staff.
• Participates in provider call schedule.
• Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning.
• Assists the IDT in formulating a plan for the coordination of 24-hour care.
• Documents participant changes appropriately in the medical record and communicates participant changes to team members.
• Evaluates and treats participants in their homes, assisted living facilities, nursing homes, and hospitals when medically indicated.
• Encourages all participants to have Advanced Directives and provides education as needed; may discuss the Do Not Resuscitate (DNR) option with the participant and/or family members and may complete the Health Care Wishes form and write DNR orders in the medical record, in collaboration with a physician.
Qualifications
• NP licensure in perspective state and a Family certification from Nurse Practitioner program.
• Only act within the scope of his or her authority to practice.
• Meet a standardized set of competencies for the specific position description established by the state and approved by CMS before working
• 1 year of experience working with a frail or elderly population preferred. If this is not present, training on working with a frail or elderly population will be provided upon hiring.
• Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
• Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
• Pass a comprehensive criminal background check that may include, but is not limited to, federal and state Medicare/Medicaid exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
