SCHEDULE:

COMPENSATION:
- $125,000 to $155,000 Annual Income
- $105,000 to $125,000 Base Salary
- $20,000 to $30,000 per Year in RVU Bonuses

LOCATION:

This practice sees patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), independent living facilities (ILFs), and private homes. They are currently hiring in the Dayton area and will work with providers during the interview process to ensure a mutually-agreeable geographic territory in which all patients / buildings will be located.


COMPANY PROFILE:
This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of where they call home. They currently boast a team of more than 15 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, transitional care, and remote patient monitoring.


POSITION DESCRIPTION:
The nurse practitioner will serve as the primary care provider for a dedicated panel of geriatric patients.

All patients are located in facilities or private homes. Depending on the territory, the practice is able to offer different mixes of facilities (ALFs / ILFs / SNFs) and home-based patients. During the initial phone interview, the practice will look at the provider's location and discuss this in detail.

In SNF settings (with no travel during the day), providers see an average of 15 to 16 patients per day.

When traveling between ALFs, ILFs, and homes, providers see an average of 9 to 12 patients per day (this is adjusted based on actual travel time).

Specific duties of the role include, but are not limited to:

- conduct health assessments to identify patient needs
- develop personalized care plans
- regularly evaluate patient conditions and adjust treatment plans as necessary
- administer medications, treatments, and other therapeutic interventions as prescribed
- perform minor procedures and wound care as required (and as settings permit)
- monitor vital signs and patient responses to treatment
- educate patients and families on health management, disease prevention, and treatments
- provide guidance on lifestyle modifications and self-care practices
- work with physicians, nurses, and other professionals to coordinate comprehensive care
- ensure compliance with all relevant healthcare regulations and standards

The practice uses the eClinicalWorks EHR.

On-call is phone triage only with very low volume. Providers rotate the on-call responsibility, with a shift every 10 weeks or less for full-time providers.



REQUIREMENTS:
- Ohio NP license
- DEA
- 1+ year advanced practice experience
- previous experience in geriatrics, LTC, SNFs, house calls, etc.
