The ideal candidate will have a strong background in geriatrics or home care, bringing expertise and empathy to the role. You will be responsible for conducting well-visits, managing chronic conditions, and collaborating with our multidisciplinary team to enhance the health and well-being of our homebound patients.
Key Responsibilities:
- Conduct Well-Visits: Perform thorough physical examinations, health assessments, and routine screenings for elderly patients during home visits.
- Patient Management: Develop, implement, and evaluate individualized care plans. Monitor chronic conditions, medication regimens, and provide education on disease management.
- Collaboration: Work closely with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated and comprehensive care.
- Documentation: Maintain accurate and timely electronic medical records, including patient history, assessment findings, and care plans.
- Patient Education: Provide education to patients and their families on health maintenance, disease prevention, and managing chronic conditions.
- Follow-Up Care: Coordinate and manage follow-up visits, laboratory tests, and referrals to specialists as needed.
- Emergency Response: Identify and respond to medical emergencies during home visits, providing initial care and coordinating further treatment as necessary.
- Quality Improvement: Participate in quality improvement initiatives, contributing to the development and implementation of best practices in home care.
Requirements:
- Master's degree in Nursing from an accredited Nurse Practitioner program.
- Licensure: Current and valid Nurse Practitioner license in NJ. Certification in Adult-Gerontology Primary Care or Family Nurse Practitioner preferred.
- Experience: Minimum of 1 year of experience in geriatrics or home care settings.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal abilities.
- Compassionate approach with a focus on patient-centered care.
Please note- The annual base salary range is $140k to $155k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
