Provides primary medical care services to older adults, using primary care concepts,
emphasizing prevention of illness and the promotion of healthy lifestyles.
Provides leadership and training to other health care professionals. Functions as a member of the PACE Interdisciplinary Team (IDT) in care coordination of participants; participates in the program planning of health activities within the health Center and in the community, as requested.

Job Responsibilities
- Conduct comprehensive patient assessments including vital signs monitoring.
- Provide urgent care services and treatment to patients in need.
- Utilize electronic health record systems such as Cerner for accurate documentation.
- Collaborate with physicians and healthcare professionals to deliver optimal patient care.
- Administer specialized treatments in settings like assisted living facilities.
- Demonstrate in-depth knowledge of human anatomy and physiology in clinical practice.
- Operate advanced medical equipment like neonatal ventilators and dialysis machines.
- Perform procedures such as spinal taps and manage critical care situations effectively.
