Seeking a patient-centered and empathetic AGCNP or FNP to join PACE program team. This individual will provide primary medical management in collaboration with a physician.

The Program of All-Inclusive Care of the Elderly (PACE) is designed to help our participants live more independently by offering a wide range of medical, functional, emotional, cognitive, and social services. PACE is designed to keep the participant living in the community, preferably in their home, as long as it's safe to do so. Each PACE participant has a custom care plan designed with services and care staff to cover needed services from primary care to rehabilitation services and more.

Position Details:

  • Functions as a member of the PACE interdisciplinary team for collaborative decision making for the participants' plan of care.
  • Spend at least 60% of the time working on coordination of care for each participant that includes detailed chart reviews, appropriate ordering of services, and identifying barriers that may lead to over utilization of services.
  • Team: RNs, LPNs, MAs, Physical Therapy, Occupational Therapy, Activities Coordinator, Social Worker, Community Liaison
  • PACE Center open M-F, 8 a – 5 p
