Nurse Practitioner (Geriatric) - Philadelphia, PA

Job Responsibilities

1) The primary purpose of your job position is to provide primary medical management in collaboration with the PACE Organization (PO) Physician(s).
2) Performs assessments in collaboration with the physician and as needed.
3) Develops and implements appropriate plans of care to PO participants.
4) Evaluates participant physical complaints and provides appropriate treatment.
5) Provides participants and caregiver teaching and education.

Qualifications

1) ANCC/AANP Board Certified - REQUIRED
2) Active PA license - REQUIRED
3) 5-7 years of NP experience preferably in long-term care and minimum 1 year working with the frail or elderly population - REQUIRED
4) BLS - REQUIRED
5) CDS Certification - REQUIRED
6) DEA Registration - REQUIRED

Skills
