Primary Care PACE Nurse Practitioner

Join our robust team as a primary care provider for a San Diego based non-profit organization providing high quality, compassionate primary care in an outpatient setting.

Opportunity for Loan Repayment or Loan Forgiveness!

Description
-Adult Medicine provider managing acute and chronic conditions
-Refer participants to other in-house providers, including behavioral health
-Review and manage imaging and lab results
-Prescribe medications for acute and chronic conditions
-Document visits in the electronic medical record
-Possible SNF and home visits

Hours
-Monday through Friday
-Full time

Location
San Diego, California

Requirements
• 2 years NP experience, post completion of clinical training
• Board certification

Preferred
• Employment experience in a community health center
• Leadership experience a plus!
