Job Summary:
We are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team and provide virtual care to patients enrolled in our Remote Patient Monitoring program. The NP will assess, diagnose, and manage chronic conditions while collaborating with an interdisciplinary team to deliver patient-centered care. Key Responsibilities:

Conduct virtual consultations and follow-up visits with patients enrolled in RPM services.

Review and interpret real-time patient health data (e.g., vitals, glucose levels, blood pressure) to identify trends and potential concerns.

Develop and implement treatment plans based on clinical assessments and patient needs.

Educate patients on disease management, medication adherence, and lifestyle modifications.

Provide timely documentation of all patient interactions and maintain accurate electronic health records (EHR).

Monitor and escalate critical patient data to the appropriate medical team when necessary.

Stay up to date with best practices, regulations, and guidelines in telemedicine and remote patient monitoring.
Qualifications:

Active Nurse Practitioner (NP) license in the United States.

Board-certified in Family Practice, Adult-Gerontology, or another relevant specialty.

Minimum of 2 years of clinical experience (experience in telemedicine, chronic disease management, or remote patient monitoring is a plus).

Proficient in using digital health technology, electronic medical records (EMR), and virtual communication tools.

Strong clinical judgment and ability to make evidence-based decisions.

Excellent communication, interpersonal, and patient engagement skills.

Ability to work independently while collaborating effectively within a remote healthcare team.

Comfortable with technology and adapting to new healthcare innovations.
