POSITION:
Family Nurse Practitioner

RESPONSIBILITIES: The Family Nurse Practitioner provides generalized health care services; is under the supervision and reports directly to the supervising licensed physician. The Nurse Practitioner assumes principle responsibility, functioning with approved protocols for health maintenance and clinical management of acute and chronically ill patients. Identifies the health status of patients by taking medical histories, performing examinations and initiating and evaluating screening and diagnostic procedures. Initiates appropriate therapeutic intervention based upon assessment and evaluation of the health status of the individual. Determines which cases to refer to the licensed physician for further diagnosis and treatment; serves as clinical consultant to nursing and paramedical personnel; and coordinates work activities with families, community agencies and other members of the health. The Family Nurse Practitioner is a member of the patients care team and is involved in the coordination of patient care. Participates in annual trainings, such as: CME and the Patient Centered Medical Home (PCMH). Performs other duties as assigned.



Qualifications

REQUIREMENTS: Mature judgement, tact, discretion and confidentiality are mandatory. Requires a professional, pleasant and effective representation of the corporation. Bilingual (Spanish/English) preferred.

OTHER REQUIREMENTS: Must possess a dependable automobile with insurance and a valid California drivers license. Must submit to and pass a urinalysis test, fitness for duty, criminal background and credentialing clearance as a precondition for employment. Must be able to provide proof of COVID-19 vaccination and booster, recent TB test, MMR, TDAP and Hepatitis B vaccines. BLS/CPR certification required.

QUALIFICATIONS: Must be a certified Licensed Nurse Practitioner. An RN with at least a Master Degree in Nursing and advanced education in primary care.
