Job Description
We are seeking compassionate and skilled Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to join our innovative care team. As a permanent member of our organization, you’ll play a vital role in transforming patient care by delivering personalized, high-quality medical services to individuals living with chronic kidney disease (CKD) and end-stage renal disease (ESRD/ESKD).

Salary: $120,000.00 - $140,000.00

Float Position:
• Doing in-home visits within the Houston pod (not assigned to a specific territory).
• Visiting hospitals to engage and help coordinate care/discharge plans.
• No limit on # of visit/day but it will be within an 8:30am - 5pm shift.
• Drive time up to 1hr to 1.5 hr. Min. of 3 days/week in home visits, must be flex to do up to 5 days/week
