AdventHealth Well 65+ is seeking an experienced Advanced Practice Registered Nurse or a Physician Assistant to join a busy, value based care, primary care practice in Oviedo, Florida! We offer a multi-disciplinary approach to primary care for members 65 and older, dedicated to providing members “whole person care” for all health and well-being needs in one location.

Job Responsibilities
Since its inception in 2021, AdventHealth Well 65+ has consistently stayed in the 95%+ range for patient satisfaction. Fulfill your calling at AdventHealth Well 65+ and keep patients healthier and happier to enjoy more of what life has to offer in the beautiful sunshine state.
At work, you will experience:
• More time with patients. Up to 40 minutes with patients to answer questions and address concerns.
• Smaller patient panel. Max patient panel of 200 and assist with acute and TOC patients.
• Lower Daily Patient Census. 14-16 patients/day
• Interdisciplinary team. Includes RN Case Management, 1.5 Health Coaches (MA’s) per provider, Social Worker, Pharmacy, Care Coordinators, Welcome Coordinator, HCC Coding Support, and Administrator.
• Collegiality. Work in a team-oriented environment with like-minded physicians in offices housing 4-6 providers.
• Full Spectrum Utilization. You will have the opportunity to practice medicine as it should be and utilize all of your clinical skills-preventive care, chronic disease management, acute care, procedures, resulting in lower number of appropriate specialty consults.
• Environment. Our offices are new and intentionally designed to meet the needs of our geriatric population. Large Wellness suits, huddle rooms, community rooms, and more.
• Community. Beautifully appointed community rooms in each clinic to foster a social environment with activities, refreshments, and learning opportunities.
Qualifications
• Master’s degree in Nursing (FNP or APRN) or Physician Assistant Studies
• Must have 2+ years of clinical practice experience as an APRN or PA-C
Licensure, Certification, and Registration:
• Current, valid Florida APRN or PA-C license
• Current, valid DEA license
• ANCC, AANP or NCCPA (or equivalent) certification
• Current BLS required; ACLS preferred
