CHI Health Clinic is actively seeking a full-time Family Medicine Nurse Practitioner for our Diabetes Clinic to join our established practice at our Immanuel Family Health Center Omaha, Nebraska. CHI Health Clinic has primary care clinics across Nebraska and Southwest Iowa

The Advanced Practice Provider (APP) is responsible for providing direct clinical care in an ambulatory setting and provides health maintenance exams with appropriate patient education for all age groups including well child exams, adolescent exams, sports physicals and adult physicals including pap and pelvic exams. They will perform procedures commensurate with education, training and experience. Our Advanced Practice Providers are a collaborative member of an interdisciplinary care team with advanced communication skills.
Responsibilities
CHI Health Diabetes Clinic is for patients with diabetes who need ongoing diabetes management and check ups. Our specialized Nurse Practitioners help with medication and technology management for patients with diabetes. The diabetes clinic has a collaborative approach with a patient's Primary care provider, a clinical pharmacist, diabetes education and the nursing team. Our interdisciplinary approach allows us to provide frequent check-ins with patients and help them reach their health and wellness goals. Our Nurse practitioners prescribe and manage insulin pumps for patients with diabetes. Our pump program will help you explore pump options and give you the confidence to manage your insulin pump successfully.

- Average of 20-24 patient visits per day
- 4 or 4 1/2 day work week Monday - Friday

Qualifications
- Minimum of one year experience as a Family Nurse Practitioner
- Master's Degree in Family Nurse Practitioner from accredited program
- Certification by a nationally recognized certification organization
- Diabetes Educator experience highly desired
- CDCES Certification highly desired
- Nebraska License required
- Nebraska DEA required
- BLS
Pay Range
$56.36 - $65.76 /hour
