A leading healthcare organization in North Carolina is currently seeking a Primary Care Nurse Practitioner to join their team on a part-time basis. This field-based role focuses on conducting comprehensive initial patient assessments and delivering high-quality care to individuals with complex medical conditions.
About the Opportunity:
- Hybrid or Remote: In-person (50% in-person, 50% telemedicine, transitioning to more in-person)
- Shift: Flexible
- Schedule: Part-time, 20 hours per week (2.5 days per week)
- Setting: Field-based (home visits, clinic, mobile medical unit)
- Start Date: ASAP
- Travel: Required within Greenville, NC, and surrounding counties (mileage reimbursement provided)
Responsibilities:
The Primary Care Nurse Practitioner will be responsible for:
- Conducting initial patient visits to establish care and build strong relationships.
- Performing comprehensive physical exams, intakes, and assessments to determine appropriate care plans.
- Providing care in various settings, including patient homes, a clinic, or a mobile medical unit.
- Collaborating with a multidisciplinary team, including social workers, behavioral health providers, and specialists.
- Transitioning primary care patients to their continuity provider for long-term management.
- Assisting with urgent care and triage visits as needed.
- Identifying and addressing barriers to care, including financial, social, and environmental challenges.
- Supporting population health initiatives by contributing to improvements in the care model.
Qualifications:
- 2+ years of clinical experience in primary care, chronic care, or complex care.
- Active Nurse Practitioner (NP) license in North Carolina or ability to obtain by start date.
- Completed an accredited NP program with certification to practice.
- Strong patient engagement skills with a passion for building relationships.
- Experience conducting home visits and working independently in the field.
- Ability to travel within the assigned territory with reliable transportation.
- Comfort with basic procedural skills in a primary care setting.
- Fully vaccinated with all necessary vaccinations per safety requirements.
Desired Skills:
- Experience managing new patient intakes and establishing continuity of care.
- Background in mobile healthcare delivery or home-based patient care.
- Strong problem-solving skills with an innovative approach to care delivery.
