We are seeking a dedicated full-time Family Medicine Nurse Practitioner to join our value-based care practice in Marietta, GA. In this role, you will have the opportunity to deliver whole-person care by focusing on quality over quantity, meeting with 10 to 15 patients daily for 30- to 60-minute appointments. You will be supported by a robust care team, including a health coach, to help patients achieve their health goals and improve outcomes.

Compensation & Benefits: $115,000 - $138,000 & bonus eligible. Premium medical, dental, vision coverage with health savings account pre-tax savings option. Key Responsibilities:

  • Provide comprehensive, patient-centered primary care for individuals and families.
  • Conduct 30- to 60-minute appointments focusing on prevention, chronic disease management, and population health.
  • Collaborate with an interdisciplinary care team, including health coaches, to ensure coordinated and holistic patient care.
  • Utilize EMR systems (NextGen preferred) to maintain accurate and timely documentation and ensure coding compliance, including Medicare Advantage and HCC.
  • Engage in proactive case management and population health strategies to improve patient outcomes.
  • Leverage experience in team-based care and Google Workspace to enhance efficiency and communication.

Requirements:

  • Active Nurse Practitioner Board Certification in Family Medicine, Primary Care, or Internal Medicine.
  • Current, valid, and unrestricted medical or ARNP license, along with a valid DEA number.
  • Minimum of 3-5 years of primary care experience.
  • At least 2+ years of experience in case management, focusing on health prevention and population health.
  • Proficiency in EMR systems, with NextGen experience preferred.
  • Expertise in coding and documentation, especially with Medicare Advantage and HCC.
  • Familiarity with Google Workspace tools for collaboration and documentation.
