Salary and Benefits
• Base Salary: $130,000 with the potential to earn over $143,000 annually
• Performance Bonuses: Up to 10% of base salary based on patient satisfaction ratings, living the company’s values, HEDIS compliance, and reducing hospital sick days

• Pre-tax and Roth Retirement Contributions: 4.5% matching funds on 6% contributions

Responsibilities
• Provide primary care services in an outpatient setting, focusing on seniors with 3-5 complex diseases (e.g., heart disease, diabetes, hypertension)
• Value-Based Care Model with a panel size of up to 650 patients
• Average 10-12 patients per day
• Work Monday-Friday, 8 AM to 5 PM
• Participate in call schedule based on the number of providers in the market (e.g., 1:32 weeks for 32 providers)
• Collaborate with a dedicated care team, including an MA, scheduler, referral specialist, and full support from leadership

Requirements
• Current, unrestricted, active Nurse Practitioner license
• 2+ years of outpatient primary care experience required
• Experience in geriatrics is a plus but not required
• Current DEA registration
• Proficiency in basic office procedures such as suturing, joint injections, skin biopsies, and wound care
