Healthcare United is seeking a Community-Based Nurse Practitioner (NP) to join our growing team at an enhanced facility near Fairborn, Ohio. This role focuses on providing urgent and primary care to members in their homes and communities, ensuring smooth transitions of care, preventing readmissions, and supporting chronic disease management in collaboration with the primary care and interdisciplinary care teams.


Position Highlights

As a Community-Based Nurse Practitioner, you will:

  • Provide community-based medical and care coordination services for members recently discharged from the hospital.
  • Prevent hospital readmissions by addressing urgent clinical needs and managing chronic diseases.
  • Conduct home-based patient assessments, implement care plans, and educate members and their families.
  • Collaborate with primary care providers, transitional care managers, and the interdisciplinary team to address the medical, mental, and social needs of members.
  • Document care and outcomes in the Electronic Medical Record (EMR) while leveraging critical information gathered from the member’s home environment.
  • Work to remove barriers to care, ensuring members can transition seamlessly back into the medical center or other care settings.


Duties & Responsibilities

  • Discharge Coordination: Partner with transitional care managers and care teams to follow discharge plans, monitor medications, and provide education.
  • Preventive & Chronic Care: Support members with chronic disease management and implement strategies to avoid unnecessary hospitalizations.
  • Urgent Care Management: Address urgent clinical needs to prevent Emergency Room visits.
  • Home-Based Interventions: Perform comprehensive medication reviews, assess member environments, and coordinate home care services as needed.
  • Team Collaboration: Serve as a liaison between members, their families, and the primary care team to develop and implement care plans.
  • Community Engagement: Leverage local resources and build relationships to better address members’ needs


    Working Conditions

  • Operates in patient care settings with exposure to blood, bodily fluids, and infectious materials.
  • Frequently uses general office equipment and wears protective attire, including lab coats, gloves, and masks.
  • High patient traffic and shared workspaces are common.

Physical Requirements

  • Ability to remain stationary for extended periods.
  • Perform repetitive movements and occasionally lift/move equipment or supplies weighing up to 20 lbs.
  • Adapt to physically demanding tasks in a community-based care environment.



Requirements


  • Active Nurse Practitioner License.
  • 2+ years of NP experience preferred, with exposure to multiple care settings (e.g., urgent care, ER, inpatient, community-based).
  • Familiarity with care transitions, reducing hospital readmissions, and managing chronic conditions.
  • Experience with high-risk, medically complex patients and comorbidities is highly preferred.
  • Psychiatric and substance use disorder experience is helpful.
  • Knowledge of local geography, population, and resources.
  • Proficiency with Electronic Medical Records (EMR) and strong computer skills (e.g., Microsoft Office).
  • Excellent interpersonal and communication skills.
  • Ability to thrive in a fast-paced, patient-centered, results-oriented environment.
