Job Title: Physician Assistant / Nurse Practitioner - Chicago, IL

Job Type: Full-time hours may vary including evenings and weekends. On-call shared with other providers (1:4)

Reports To: Manager of Clinical Operations / Chief Medical Officer (CMO)

Responsibilities:

  • Understanding and willingness to deliver care in a primary care setting.
  • Prescribe appropriate pharmacologic and non-pharmacologic treatments.
  • Implement interventions in attempts to restore the patient's desired physiologic status.
  • Monitor the effectiveness of interventions
  • Make efforts to correctly diagnose common and less common ailments.
  • Investigates clinical problems and offers sound medical solutions.
  • Communicates effectively with other healthcare team members, patients, family members, and healthcare designees
  • Performs procedures utilizing medically approved approaches and equipment appropriate to the patient's age and size
  • Provides information and instructions at a level of understanding appropriate to the comprehension level of the patient and/or significant others.

Qualifications:

  • Active License of Physician Assistant or Nurse Practitioner in the state of Illinois.
  • Minimum 2 years experience in the field, ideally in a Federally Qualified Health Center (FQHC) or community health center.
  • Proven experience in both adult and pediatric medicine.
  • Master's degree in Nursing or Physician Assistant Studies from an accredited institution.
  • Certification in Family Medicine (preferred) to encompass both adult and pediatric patient populations.
  • Current provider card in CPR.
  • Current DEA and Controlled Substance License when applicable.
